To create a new survey  from a saved template,

Click on the Create New Survey Option and choose how you want to start a new survey (from a predefined template or from a blank).

This will start the Survey Wizard.

Click on the My Survey Templates Tab.

• Click on  the survey that you want to use
• Then you will move to the next step in the wizard. This will open the Create Survey Window with the survey loaded and ready to edit.
